Initial appointments are mostly via telehealth. Labs and imaging are typically completed before an in-person visit. Additional telehealth appointments are available if a physical exam is needed.
- Driver’s license
- Insurance card(s)
- Up-to-date medication list
- Any relevant lab or imaging results
Preferred: OhioHealth laboratories (results accessible online before your visit). Alternative: Ask your PCP to fax results to (614) 784-1153 or bring a copy with you.
Call (614) 784-UROL (8765) or message staff through the Patient Portal. You must have been seen within the last 12 months.
Be prepared to provide:
- Name, DOB, best contact number
- Any new medications or allergies
- Effectiveness of current medication
- Scheduled follow-up appointment
- Days of medication remaining or if it has run out
- Pharmacy name, phone number, and hours
